One of the most frequently asked questions by those applying for a visa or requesting online renewal is: "What is the Request for Criminal Record Check?". Where can I find the template?

Indeed, there is no official template available on AIMA's website. In this guide, we present an authorization declaration template for renewal cases that can be used and adapted for other processes.

What Is the Request for Criminal Record Check?

The Request for Criminal Record Check is a declaration by which the applicant authorizes AIMA – Agency for Integration, Migration and Asylum – to consult their Portuguese criminal record directly in official systems.

Important

Basically, it is not necessary to present the Portuguese criminal record certificate. However, the applicant must authorize AIMA to make the consultation directly, respecting the personal data protection regime.

The requirement for this authorization is provided in article 42-D of Regulatory Decree No. 84/2007, of November 5th, and subsequent amendments. This article establishes that information on criminal records in Portugal is verified through consultation of the Portuguese criminal record information system.

Required fields

First Name(s): First name(s) as per passport

Surname (Family Name): Last name(s) / family name as per passport

Father's full name: Father's complete name

Mother's full name: Mother's complete name

Date of Birth: Date of birth (day/month/year)

Current Nationality: Current nationality

Travel Document Number: Valid passport number

Place and Date: City where you sign and complete date

Signature: Must be identical to passport signature

Attention to details

Fill in all fields correctly

Use names exactly as they appear on passport

Signature must be identical to identification document

Do not leave any fields blank

Update the year in the signature date (2025)

Can I use this template for a visa application?

Yes, this template can be adapted for visa applications. Simply change the expression "applicant for residency permit renewal" to "applicant for residence visa" or similar, depending on your specific case.

Does the declaration need notarization?

Notarization is not required for processes with AIMA. A simple declaration, properly signed as per passport, is sufficient.

What if I don't have a criminal record in Portugal?

Even if you have never had any issues with Portuguese justice, the authorization is mandatory. AIMA will make the consultation and, if there are no records, the procedure continues normally. The absence of a criminal record is one of the requirements for granting and renewal.

Does this declaration replace the criminal record certificate from my country?

No. This declaration refers only to the Portuguese criminal record. For visa applications or first residence permit, you may also need to present a criminal record certificate from your country of origin or habitual residence, with apostille and sworn translation.
